This unit describes the application of evidence-based medicine (EBM). The discussion begins with the framing of clinical questions that can be answered by appropriate evidence. It then demonstrates how to find and apply the best evidence for answering four major types of clinical questions: interventions, diagnosis, harm, and prognosis. The unit also introduces summarizing of evidence (systematic reviews) as well as clinical practice guidelines and concludes with a discussion of the limitations of EBM.

Evolocumab found to be safe and effective at reducing cholesterol in Chinese patients with type 2 diabetes

This resource is a video abstract of a research paper created by Research Square on behalf of its authors. It provides a synopsis that's easy to understand, and can be used to introduce the topics it covers to students, researchers, and the general public. The video's transcript is also provided in full, with a portion provided below for preview:

"According to a new study, the cholesterol-reducing drug evolocumab plus a moderate-intensity statin dose was effective in Chinese patients with both type 2 diabetes and hyperlipidemia or mixed dyslipidemia. China has the highest diabetes burden of any country in the world. And as is the case for Western populations, diabetes is a major risk factor for cardiovascular disease in Asian adults. The primary target for treating such patients is reducing low-density lipoprotein, or LDL, cholesterol. While statin therapy is recommended globally, research suggests that statin-related adverse reactions occur at higher rates among Chinese patients. The inability to achieve the recommended LDL levels in Chinese patients might therefore be related to statin intolerance or underuse of statins due to the risk of adverse events..."

This resource is a video abstract of a research paper created by Research Square on behalf of its authors. It provides a synopsis that's easy to understand, and can be used to introduce the topics it covers to students, researchers, and the general public. The video's transcript is also provided in full, with a portion provided below for preview:

"Diabetes is a major risk factor for myocardial infarction, stroke, and other cardiovascular diseases. The prime target for reducing cardiovascular risk is low-density lipoprotein, or LDL. LDL cholesterol has been linked to cardiovascular events in patients with type 2 diabetes. While statins have long been the primary treatment for reducing LDL, many patients are unable to reach recommended levels with a statin alone or are unable to take an effective dose. That places them at an especially high risk for cardiovascular disease. Now, a new study reports the efficacy and safety of evolocumab on top of statins in patients with both type 2 diabetes and hyperlipidemia or mixed dyslipidemia. The double-blind, phase 3 trial was conducted in patients from 10 different countries over a treatment period of 12 weeks. More than 980 patients were randomized to one of four subcutaneous treatments: 140 mg of evolocumab every 2 weeks, 420 mg of evolocumab monthly, or placebo at either of those frequencies..."

Building on the Introduction to Healthy Relationships lesson by the same authors, this lesson expands students’ understanding of healthy relationships. Students will review what healthy relationships look and feel like. Students will focus on how to assess the health of their own romantic and platonic relationships. The lesson also expands on power dynamics, boundaries, and consent as appropriate for older high school students. After an in-depth, discussion-based presentation reviewing these concepts, students will apply an evaluation process to review the health of fictional relationships in small groups.

Exploring Climate Change and Mental Health
Conditional Remix & Share Permitted

Short Description:
When it comes to climate change, studies have shown that youth are more likely to report mental health concerns in comparison to older generations. Emerging research underlines that youth who enroll in environmental classes consistently report increased levels of stress as a result of their heightened awareness of planetary health challenges. This toolkit is designed for use by educators to empower students to think critically about the structural and socio-political inequities that affect them while centering climate change and mental health through embedded reflective exercises. Questions that explore eco-anxiety, ecological paralysis and ecological grief are included in this toolkit. Students are encouraged to answer questions according to the emotions that they resonate most with and want to explore further. Additionally, students are encouraged to examine the relationship between anthropogenic activity, mental health and their values, emotions and behaviours. The PDF version of this toolkit is available for download here: https://pressbooks.bccampus.ca/eccmh/wp-content/uploads/sites/1644/2022/04/2April2022_TOOLKIT_Exploring-Mental-Health-and-Climate-Change_compressed-1.pdf.

Long Description:
When it comes to climate change, studies have shown that youth are more likely to report mental health concerns in comparison to older generations. Emerging research underlines that youth who enroll in environmental classes consistently report increased levels of stress as a result of their heightened awareness of planetary health challenges. Unfortunately, though there are increasing numbers of people who will be affected by the mental health impacts of climate change, factors such as psychological distance and denial influence the way individuals form their beliefs and take action on this issue. In response to the need for more innovative teaching tools in undergraduate curricula, as well as the eagerness of youth to learn about planetary health, a 25-minute film and accompanying toolkit were created focusing on the relationship between mental health and climate change. In addition to climate change experts, the film features UBC students who were previously enrolled in the UBC nursing elective course NURS 290: Health Impacts of Climate Change. The educational toolkit uses knowledge mobilization and health promotion strategies based on the Transtheoretical Model of Change. The film and toolkit are tools designed for use by educators to empower students to think critically about the structural and socio-political inequities that affect them while centering climate change and mental health through embedded reflective exercises. By featuring the voices of students, this project is not only relevant and impactful but also addresses the diverse learning needs of students. The PDF version of this toolkit is available for download here.

Exploring and avoiding morphine-induced spinal masses

It provides a synopsis that's easy to understand, and can be used to introduce the topics it covers to students, researchers, and the general public. The video's transcript is also provided in full, with a portion provided below for preview:

"Harmful masses that accompany regular spinal or intrathecal infusions of opiates for pain management might one day be a concern of the past. Researchers have uncovered the cellular mechanisms behind these lesions, which occur with some opioids but not others. Their findings could help improve spinal pain therapy for patients without compromising their health or quality of life. Intrathecal opioids such as morphine are highly effective for managing pain. Direct access to opioid receptors in the spinal cord has made round-the-clock pain management possible, most notably through the use of so-called “pain pumps”. One concern, however, has been the possible formation of collagen-rich masses around the area of drug delivery. These masses, or granulomas, occur in about 8% of patients receiving spinal morphine infusions and have been linked, ironically, to increased pain and reduced neurological function..."
