This project assesses a sample of California county programs for preparing foster teenagers to live independently. Counties were selected to represent statewide variability and represent northern and southern regions as well as urban and rural areas. Chapters address: the organizational structure of each program including the agencies providing ILP services, agency staffing, coordination mechanisms, foster care supervision, and community involvement; a description of program participants including characteristics of the youth, diversity, readiness for the program, barriers to participation, foster care provider issues, foster parent training, and the relationship of birth parents to the county agency and the youth; a description of program processes including identification of eligible youth, referral, outreach, assessment, out-of-county placement, monitoring and follow-up; and an overview of program content and services including classes, activities, individual services, housing issues, and aftercare support. In the same year that Wraparound was launched; “well-being” was added as a major goal of the national agenda to improve child and youth outcomes through the sweeping Adoptions and Safe Families Act (ASFA). Wraparound was originally limited to a subset of children and youth with complex needs; however; system changes aimed at promoting well-being more widely; such as the Katie A. Settlement Agreement; the Continuum of Care Reform (CCR) and Resource Family Approval (RFA); have led to universalizing Wraparound’s team-based; family-centered approach across developmental domains; major public systems and community agencies that serve children; youth and families. This presentation introduces the nation’s first integrated core practice model for the systems of child welfare; behavioral health and juvenile justice; with a blueprint for workforce development for an even wider shared practice environment in the future.

Young foster children are at high risk for mental health difficulties; which may make adjusting to a new home difficult and imperil the security of the placement. Indeed; externalizing behaviors in young children are one of the strongest predictors of placement disruption; and having one disruption increases the risk of future disruptions. In an effort to stabilize placements and reduce children’s mental health difficulties; we offer Parent-Child Care (PC-CARE); a 6-week parenting intervention; to all newly placed foster children aged 1-5 years in Sacramento County. In this presentation; we will describe the PCCARE model and present results of the effectiveness of the program.
